Coast Guard Releases FY25 Annual Safety Report

The newly formed U.S. Coast Guard Safety Service Center (SAFECEN) has released the Fiscal Year 2025 Annual Safety Report (ASR) (CAC required).

In fiscal year (FY) 2025, the Coast Guard documented 2,933 mishaps resulting in one death, six permanent full or partial disabilities, and $25.9 million in property damage. These mishaps cost $7.6 million in workers’ compensation and led to 9,104 lost workdays and 21,673 restricted days.

The annual safety report provides Coast Guard communities with data on mishap exposure, causal factors, types of losses, and identified hazards for cutters, boats, and aviation. The service has provided these mishap and hazard analyses since 2013.

This year’s report also highlights the establishment of SAFECEN as part of ongoing Coast Guard transformation efforts. The center consolidated major mishap response and occupational safety, significantly increasing safety and risk management services compared to FY 2024.

To protect personnel health and maintain mission readiness, staff conducted 655 assessments at shore, afloat, and aviation units this year. These included:

  • Safety mobile assistance, response, and training assessments
  • Industrial hygiene health risk assessments
  • Occupational safety health risk assessments
  • Ship sanitation certificate inspections

Operational Safety was central to enhancing mission readiness and success through the management of mishaps, streamlining Directed Action (DA) resolution, and fostering a culture of proactive risk management. Key components of these efforts include:

  • Advanced Mishap Analysis and Reporting Course: Five courses delivered training to 300+ students, including five Other Government Agency and four Auxiliary students.
  • Joint Safety Council (JSC): Collaborated with Department of War counterparts including briefing relevant CG mishaps and lessons learned, and engaging in joint service working group efforts to share risk-reduction initiative.

Directed Actions (DA) resolution: Maintained stakeholder engagement across programs to resolve 16 DAs and reduce the average age of unresolved DAs by nine months, marking the quickest resolution rate in over a decade. By protecting personnel from hazards and reducing operational disruptions, the Safety Service Center is the Coast Guard’s proactive solution to enhance operations and mission readiness while ensuring compliance with occupational safety and environmental health policies.
